By checking the box below, I give IGNITE Student Union (IGNITE) permission to use my name, photos, and/or audio, video, written testimonials, digital recordings, negatives, slides, prints, or other electronic images (collectively “my likeness”) to promote and advertise IGNITE’s programs and services in any format or medium, or for other related purposes, royalty-free and in perpetuity.

IGNITE may edit or annotate my likeness if needed, and I understand that I won’t be able to review or approve the final versions. My likeness might be used with or without identifying me by name or affiliation.

Should I change my mind, I may contact IGNITE via email or phone to revoke permission for using my name or any part of my likeness in the future. However, I understand this change won’t affect any materials that have already been published.

I confirm that I am over eighteen (18) years old and that I have read and understand this Consent and License. In accordance with Ontario’s Freedom of Information and Protection of Privacy Act, I consent to the collection of my personal information in the form of my likeness and name by IGNITE, its employees, agents, and representatives, to be used for the purposes described above and shared with third parties as outlined.
